Introduction

This glasses warranty program is offered by New Westminster and Suter Brook Optometry clinics along with the Community Outreach Eye Care Program to customers who have purchased prescription eyeglasses (“Product”) from our facility. This program is designed to offer protection against accidental breakage and provides a one-time replacement option in case of loss. Please read these Terms and Conditions carefully to understand your rights and obligations under this program.

Eligibility

1. To be eligible for this program, customers must have purchased the product from New Westminster Optometry Clinic, Suter Brook Optometry Clinic or the Community Outreach Eye Care Program.
2. This Program is only applicable to prescription eyeglasses and does not cover contact lenses, sunglasses, or any other accessory.
3. The program must be purchased within 1 week of the original purchase of the product.

    

Pricing

1. Single Vision Frame and Lens Warranty: Initial cost of $49
2. Progressive Frame and Lens Warranty: Initial cost of $99
3. Co-pay for frame replacement due to breakage: $39
4. Co-pay for single vision lens replacement: $39
5. Co-pay for progressive lens replacement: $99

    

Coverage

1. Accidental Breakage: This program covers one-time accidental breakage of the frame or lenses. Accidental breakage refers to unintentional physical damage that occurs under normal usage conditions.
2. Loss: In the event of loss of the product, the program allows for a one-time replacement of the same frame or another frame of equal or lesser value at a 50% cost of the original frame.

    

Exclusions

1. This program does not cover damage resulting from misuse, abuse, or neglect
2. Cosmetic damage that does not affect the functionality of the product is not covered.

    

Claim Process

1. To initiate a claim under this program, you just present the damaged or lost product along with proof of purchase and identification to New Westminster Optometry Clinic, Suter Brook Optometry Clinic.
2. All claims must be made within the warranty period, which is either one year from the date of original purchase, depending on the terms you selected when purchasing the program.

    

Limitations

1. This program allows for only one replacement of the product in the event of accidental breakage or loss.
2. The replacement frame, in case of loss, will be provided at a 50% cost based on the original frame and lens retail value.
3. No refunds will be issued for the program fee under any circumstances.

    

Changes to the Program

New Westminster Optometry Clinic and Suter Brook Optometry Clinic reserves the right to alter, modify, or terminate this program at any time, provided that such changes will not affect warranties purchased prior to the date of the change.

These Terms and Conditions form a legal agreement between you and New Westminster Optometry Clinic and Suter Brook Optometry Clinic. By purchasing this program, you agree to be bound by these terms.
